This Project Change Request Template should be filled out by the customer or employee requesting the change, then submitted to the Project Manager for assessment.
- The customer or employee requesting the change will only fill out part of the first section (Change Request Information), and the Project Manager will fill out the rest.
- This form will detail the change request and provide information regarding reasons for the change, assumptions, change priority level, and costs.
After the Project Manager makes a decision regarding the approval/rejection/escalation of the request, that decision will be recorded on this form.
