An effective change management practice requires a mechanism to capture change requests. This is accomplished through a request for change (RFC) form that includes all the important aspects to consider for analyzing and tracking new change requests. This RFC form is based on the IT Infrastructure Library (ITIL) Service Management framework and has three sections:
- Instructions and field descriptions, which serve as a guide to the person completing the RFC form.
- The RFC form where the information is captured.
- Submission instructions, which provide guidance on how to submit the RFC.
